For historical reasons we had a "using namespace std;" in port.h. This
is generally a bad idea. So removing it and along the way doing a bunch
of cpplint cleanup.

Trade a small amount of memory to improve the cache coherency of
the SymmetricRightMultiply operation.
The resulting code leads to a 10-20% speedup in the linear solver
end to end.
