Sameer Agarwal 4bf3868bec Fix a bug in the Schur eliminator
The schur eliminator treats rows with e blocks and row with
no e blocks separately. The template specialization logic only
applies to the rows with e blocks.

So, in cases where the rows with e-blocks have a fixed size f-block
but the rows without e-blocks have f-blocks of varying sizes,
DetectStructure will return a static f-block size, but we need to be
careful that we do not blindly use that static f-block size everywhere.

This patch fixes a bug where such care was not being taken, where
it was assumed that the static f-block size could be assumed for all
f-block sizes.

A new test is added, which triggers an exception in debug mode. In
release mode this error does not present itself, due to a peculiarity
of the way Eigen works.

Thanks to Werner Trobin for reporting this bug.

Keir Mierle 7492b0d8de Update copyright headers with new year and URL
Since Ceres is moving to using GitHub for issues, and the Google
Code URL in the current copyright header will soon become invalid,
update all the headers.

Sameer Agarwal bcc865f81c Remove using namespace std;
For historical reasons we had a "using namespace std;" in port.h. This
is generally a bad idea. So removing it and along the way doing a bunch
of cpplint cleanup.

Sameer Agarwal c4a329155c Enable support for dumping trust region minimizer problems.
This support was broken due to the TrustRegionMinimizer refactoring.
It is now enabled again, with the responsibilty for dumping the
problem shifted to the individual TrustRegionStrategy.

There is however one wrinkle, which is perhaps an indication of
poor design to start with. The LinearLeastSquaresProblemProto
carries in it num_eliminate_blocks, something which does not
exist anymore. More importantly, the TrustRegionStrategy does not
have access to this quantity anymore.

Dealing with this will be the subject of a future change.

Sergey Sharybin b53c9667f5 Solve No Previous Prototype GCC warning
In some cases there were missing includes of own
header files from implementation files.

In other cases moved function which are only used
within single file into an anonymous namespace.

Sameer Agarwal 4a6cc1c410 Various fixes repored by Markus Moll.
1. Termination type was using == instead of =.
2. LevenbergMarquardtStrategyTest was using an object
which was not returning the correct value.
3. DumpLinearLeastSquaresProblemToTextFile was passing an
   unnecessary argument to StringAppendF.

Sameer Agarwal 82f4b88c34 Extend support writing linear least squares problems to disk.
1. Make the mechanism for writing problems to disk, generic and
controllable using an enum DumpType visible in the API.

2. Instead of single file containing protocol buffers, now matrices can
be written in a matlab/octave friendly format. This is now the default.

3. The support for writing problems to disk is moved into
linear_least_squares_problem.cc/h

4. SparseMatrix now has a ToTextFile virtual method which is
implemented by each of its subclasses to write a (i,j,s) triplets.

5. Minor changes to simple_bundle_adjuster to enable logging at startup.
